Dorsal Root Ganglion (DRG) Stimulation for Focal Neuropathic Pain
Dorsal root ganglion (DRG) stimulation is a form of implanted neuromodulation used for selected chronic neuropathic pain conditions. The dorsal root ganglia are structures located near the spinal nerve roots that contain sensory nerve-cell bodies and play an important role in pain signaling.
DRG stimulation places leads near selected dorsal root ganglia and delivers programmed electrical stimulation intended to modulate pain signaling. Because specific DRG levels correspond to particular body regions, the therapy can be useful when the pain distribution is relatively focal.
How Is DRG Stimulation Different From Spinal Cord Stimulation?
Both DRG stimulation and conventional spinal cord stimulation (SCS) are neuromodulation therapies, but the stimulation targets are different. Conventional SCS places leads in the epidural space over the spinal cord, while DRG stimulation targets selected dorsal root ganglia. Neither approach is automatically better for every patient. The choice depends on the diagnosis, pain distribution, prior treatment, anatomy, goals, and trial response.
Conditions That May Be Considered
- Complex regional pain syndrome (CRPS)
- Selected focal postsurgical neuropathic pain
- Selected post-traumatic or causalgic pain syndromes
- Other carefully selected focal neuropathic pain conditions when supported by the clinical evaluation
DRG stimulation is not a general treatment for every case of sciatica, diabetic neuropathy, or nonspecific chronic pain.
Evaluation, Trial, and Implant
The process begins with a detailed review of the diagnosis, pain pattern, prior treatments, imaging, medical history, and treatment goals. Psychosocial screening may be part of the evaluation. When appropriate, a temporary stimulation trial is performed before permanent implantation. The trial allows the patient and physician to evaluate pain relief, function, and whether the therapy meaningfully addresses the targeted pain.
If the trial is clinically successful and the patient remains an appropriate candidate, permanent leads and an implanted pulse generator may be placed. Programming and follow-up are important parts of long-term care.
Potential Risks
Potential risks include infection, bleeding, pain at the implant site, lead migration or fracture, unwanted stimulation, device malfunction, inadequate pain relief, neurologic injury, and the possibility of revision or removal. Individual risks are reviewed before treatment.
DRG Stimulation at Solutions in Pain
Dr. David Nwokeabia is board-certified in Anesthesiology and Pain Medicine and fellowship-trained in interventional pain medicine. At Solutions in Pain, DRG stimulation is evaluated as part of a comprehensive neuromodulation program serving patients in Allen, Wylie, Collin County, and the Dallas–Fort Worth area.
Chronic pain can disrupt every aspect of your life, making even simple tasks feel overwhelming. At Solutions in Pain, we understand the challenges of living with persistent discomfort and offer advanced solutions to help you regain control. Among these options is the Dorsal Root Ganglion (DRG) Stimulation—an innovative treatment designed to provide lasting relief and improve your quality of life.
What Is the Dorsal Root Ganglion Stimulator?
The Dorsal Root Ganglion (DRG) Stimulation is a form of implanted neuromodulation for selected chronic neuropathic pain conditions, directly targeting the dorsal root ganglia, clusters of nerves responsible for transmitting pain signals. By disrupting these signals before they reach the brain, DRGS effectively alleviates pain, even for individuals who have not found relief with traditional therapies.
How Does the DRGS Work?
The DRGS system involves the implantation of a small, sophisticated device near the dorsal root ganglia. This device sends gentle electrical impulses to modulate pain signals, providing relief tailored to your specific condition. The minimally invasive nature of the procedure requires individualized recovery guidance, and clinical response varies by patient.
Conditions Treated by DRGS Include:
- Complex Regional Pain Syndrome (CRPS): Often challenging to treat, DRGS offers targeted relief for this neuropathic pain.
- Phantom Limb Pain: DRGS effectively minimizes pain experienced after limb amputation.
- Post-Surgical Pain: Persistent pain following surgery can significantly improve with this therapy.
- Peripheral Neuropathy: DRGS addresses nerve pain caused by diabetes or other conditions.
- Sciatica: By targeting nerve roots, DRGS reduces the debilitating effects of sciatica.
The Benefits of the Dorsal Root Ganglion Stimulator
Choosing DRGS can transform your approach to managing chronic pain. Here are some key benefits to consider:
- Targeted Pain Relief: DRGS focuses on specific nerve clusters responsible for your pain, ensuring precise results.
- Minimally Invasive Procedure: With just a small incision, this treatment minimizes recovery time and discomfort.
- Customizable Therapy: The device’s settings can be adjusted to match your individual needs, ensuring optimal relief.
- Revision or Removal: The system can sometimes be revised or removed if clinically necessary, but these are additional procedures with their own risks.
- Improved Quality of Life: Many patients report reduced reliance on medications and an increased ability to participate in daily activities.
Moreover, DRGS can help restore your confidence and independence by significantly reducing chronic pain’s impact on your lifestyle.
Are You a Candidate for DRGS?
You may be an ideal candidate for DRGS if:
- You suffer from chronic pain that hasn’t responded to conventional treatments.
- Your pain is neuropathic, such as in conditions like CRPS or phantom limb pain.
- You seek a minimally invasive, non-opioid alternative for pain relief.
At Solutions in Pain, our specialists perform thorough evaluations to determine if DRGS is right for you. This includes reviewing your medical history, understanding your pain patterns, and performing a physical exam.
What to Expect During DRGS Treatment
Understanding what lies ahead can make the process more comfortable and reassuring. The DRGS procedure typically includes three main steps:
- Initial Consultation: During your visit, our team will explain the DRGS procedure, answer your questions, and outline the potential benefits and risks.
- Trial Phase: A temporary device is placed to assess how well DRGS alleviates your pain. This phase ensures that you experience noticeable relief before proceeding with the permanent implant.
- Permanent Implantation: If the trial is successful, our team will implant the permanent device under local anesthesia. This minimally invasive procedure takes only a short amount of time, allowing you to return home the same day.
After the procedure, our team provides detailed recovery guidance, including activity restrictions and follow-up appointments to monitor your progress.
Long-Term Effectiveness and Results
The DRGS system offers lasting benefits, making it an excellent choice for individuals with chronic pain. Studies have shown that many patients experience substantial pain relief for months or even years after the procedure. By targeting the source of your pain, DRGS restores your ability to enjoy life without constant discomfort holding you back.
Transitioning to a life with better pain control and functionstyle takes time, but with DRGS, many patients find themselves able to pursue hobbies, spend quality time with loved ones, and reduce their dependence on medication.

Clinical Evidence, Limitations & Patient Selection
The strongest randomized evidence for DRG stimulation is in lower-extremity CRPS I/II/causalgia. The ACCURATE trial found higher treatment success with DRG stimulation than conventional SCS in that studied population.
Important limitation: Evidence for other focal neuropathic pain conditions is smaller and some uses may be off-label. DRG stimulation should not be marketed as a universal treatment for sciatica or generalized neuropathy.
Every procedure has potential risks, and individual benefit cannot be guaranteed. Recommendations at Solutions in Pain are based on the diagnosis, examination, relevant imaging, prior treatment, medical history, medications, patient goals, current evidence, and applicable payer requirements.
