Long-term opioid therapy has important risks and should be used selectively with ongoing assessment of pain, function, safety, and alternatives. When a reduction is clinically appropriate, tapering should generally be collaborative and individualized rather than abrupt. Treatment decisions also differ for cancer-related pain, palliative care, sickle cell disease, and opioid use disorder.
