Advanced Back Pain Treatment Is Diagnosis-Specific
“Back pain” is a symptom, not a single diagnosis. Treatment depends on whether the likely pain generator is a spinal nerve root, facet joint, sacroiliac joint, vertebral fracture, persistent neuropathic pain after prior surgery, muscle/myofascial pain, or another condition.
- Rehabilitation and conservative care: exercise-based therapy, activity modification, and nonopioid medication may be appropriate first steps for many patients.
- Epidural steroid injection: considered primarily for selected radicular pain/radiculopathy associated with nerve-root inflammation; it does not remove a disc herniation or mechanically decompress stenosis.
- Medial branch block and RFA: used in a diagnostic pathway for selected facet-mediated axial back pain, not routine sciatica.
- SCS/DRG neuromodulation: may be considered for selected chronic neuropathic pain after appropriate evaluation and, for most SCS indications, a screening trial.
- Other diagnosis-specific procedures: SI-joint injection, vertebral augmentation for selected painful compression fractures, or other interventions may be appropriate when supported by the diagnosis.
Structural disease that is unlikely to respond to an injection or neuromodulation may require surgical evaluation. Solutions in Pain does not present a generic “spinal decompression” treatment as a cure for nonspecific back pain.
