Reporting a concern

To report a possible error, identify the page and the specific statement or image at issue, then use the Contact page or call the office. Supporting sources or context are helpful but not required.

This website is not an emergency communication channel and this policy does not promise a response time. For a medical emergency, call 911 or seek immediate emergency care.

How concerns are assessed

  • Record the page, reported concern, date received, and supporting information.
  • Determine whether the issue is clerical, presentational, substantive, clinically material, or outside the scope of the website.
  • Route clinically material questions to an authorized clinician without representing review as complete before it occurs.
  • Check the relevant evidence, approved source record, image, and prior governed revision.
  • Document the review and authorization supporting any material correction.

How corrections are presented

A confirmed material correction may receive a visible note describing what changed and when. A substantive revision may update the displayed revision date after review. Minor spelling, grammar, layout, or formatting changes generally do not receive a clinical update date.

When correction is not sufficient, content may be consolidated, withdrawn, noindexed, or removed after appropriate owner and clinical review. A redirect is used only when there is a genuinely relevant successor page.

Medical review and version integrity

A material change to claim-bearing text, evidence, clinical imagery, or medical SEO metadata invalidates the prior exact-version clinician approval. The revised candidate must complete the applicable evidence, independent-review, deterministic, and clinician-review steps before it replaces approved content.

WordPress revision history and repository history preserve the development record. Public correction notes are not fabricated, and the absence of a public correction log does not mean that minor maintenance has never occurred.
